Full Description
The Finnish company's solutions allow users to do a simple self-assessment by using validated indicators to determine the need for health and well-being promotion activities. The users can easily assess their daily abilities, get guidance on lifestyle changes and consider the need for everyday support from social welfare and health care providers.

The self-assessment tool can also be utilized at the beginning and end of a therapy or rehabilitation period to assess the progress and effectiveness. The visualized progress report gives the users motivation.

Healthcare professionals and authorities can use the collected self-assessment data to make human-centered decisions regarding future needs of care and resources.

The company is looking for a role e.g. in a Horizon Europe project as well as pilot environments from public or private social and healthcare organizations, e.g. which are providing preventive support for youth and older adults/elderly.

Distribution partners and integrations in Health Information System can be negotiated, as well as subcontracting e.g. in related public procurements.

The company already has a few significant references in the public sector in Finland (youth and elderly).
